True Precision Rehab LLC ("True Precision Rehab", "we", "us", or "our") operates this website at www.trueprecisionrehabchicago.com. This Privacy Policy explains what information we collect through the website, how we use it, who we share it with, and the choices you have.
This policy covers the website only. If you become a patient, the records we create about your care are protected separately under federal and Illinois health privacy law and are described in the Notice of Privacy Practices we give you at your first visit.

Information you give us
We collect the information you enter into forms on this website. Depending on the form, this may include your first and last name, your email address, your phone number, your reason for wanting to visit us, what your symptoms stop you from doing, and anything else you choose to write in a message.
Some of that information describes your health. Please share only what you are comfortable putting into a web form, and never use this website to send urgent medical information.

Hosting and site analytics. This website is built and hosted on Wix. Wix sets cookies needed for the site to work and collects visitor statistics on our behalf.
Google advertising. We use Google Ads conversion tracking and remarketing. When you visit, Google may receive the address of the page you viewed, your IP address, and an advertising identifier stored in your browser. Many pages on this site describe specific conditions and treatments, so a page address on its own can suggest what you may be interested in. We do not sell your personal information. We do not share the contents of your form submissions, including anything you tell us about your symptoms, with advertising companies.

Do Not Track. Some browsers can send a "Do Not Track" signal. There is no agreed industry standard for responding to that signal, and this website does not currently respond to it.

We use the information described above to respond to your appointment requests, questions and call back requests; to send you the guides and materials you ask for; to schedule, confirm and follow up on appointments; to run and improve the website; to measure which pages and advertisements bring people to us; to send you information about our services where you have agreed to receive it; to keep the website secure and prevent misuse; and to meet our legal and professional obligations.
How we share your information
We share personal information only in these situations. With service providers who run the website, handle our forms, deliver our email, schedule appointments, and provide analytics and advertising services, acting on our instructions. With our professional advisors, such as our lawyers, accountants and insurers, where needed. For legal reasons, where we are required by law, subpoena or court order, or where it is necessary to protect the rights, safety or property of our patients, our staff or the public. And in a business transfer, if our practice is sold or reorganised, in which case information may transfer as part of that transaction.
We do not sell your personal information, and we do not rent or trade your contact details.
Email, calls, and text messages
If you give us your phone number or email address, you agree that we may contact you about your enquiry using the details you provided, including by phone, email and text message. Message and data rates may apply.
You can stop marketing email at any time using the unsubscribe link in any message. You can ask us to stop calling or texting by telling the caller, by replying STOP to a text, or by emailing info@trueprecisionrehab.com. We may still contact you about an appointment you have already booked or about your care.
How long we keep information
We keep website enquiries for as long as we need them to respond and for a reasonable period afterwards. If you become a patient, your records are kept for the period required by Illinois law and our professional obligations. Analytics and advertising data are kept for the retention periods set by the relevant service.
Security
We use reasonable administrative, technical and physical safeguards to protect the information we hold, including an encrypted connection to this website. No website or method of transmission is completely secure and we cannot guarantee absolute security. Please do not send sensitive health details, insurance identifiers or payment card numbers by web form or email.
Children
This website is intended for adults. We do not knowingly collect personal information online from children under 13. If you believe a child has given us information through this website, contact us and we will delete it. Parents and guardians may of course request appointments on behalf of a child.
Illinois residents
We do not collect biometric identifiers or biometric information through this website, as those terms are defined in the Illinois Biometric Information Privacy Act. We do not collect fingerprints, voiceprints, retina or iris scans, or scans of hand or face geometry. If that ever changes we will update this policy and follow the notice, written consent and retention requirements that apply. If a breach of personal information occurs, we will notify affected Illinois residents as required by the Illinois Personal Information Protection Act.
California residents
If you live in California you may request, once per calendar year, a list of the third parties to whom we disclosed your personal information for their own direct marketing purposes in the previous year. We do not currently make such disclosures. You may also ask what categories of personal information we collect and how we use them. Send requests to info@trueprecisionrehab.com.

This website links to other websites, including our social media pages and our online booking. We do not control those sites and we are not responsible for their content or their privacy practices. Please read their privacy policies before giving them information.
